This form, when submitted, will be confidentially received by our business Manager to investigate and address for you.  You can expect to hear from the Manager within the timeframes of our complaint policy (available on this website) and assured that we will keep you updated as to its' handling and the outcome of each and every stage of the review. We are committed to our patients and their experience at our clinic, where we strive to adopt industry best practices as a minimum standard for delivery of our services.
