I acknowledge I have read & understood both the 'Complaints Policy' and 'Patient Conduct Policy' for Embrace Medical Group t/a The DOCTORS Hope Island and agree to my complaint being handled in accordance with them.

I declare that the information provided in this complaint is true & accurate to the best of my recollection and acknowledge the clinic's CCTV Policy (accessible on this website or by request) and hereby consent to the CCTV visual & sound recordings via the Medical Centre's monitoring systems of the common areas/public spaces of its' premises to be reviewed and relied upon by management when investigating my complaint (please refer to our CCTV Policy on this website)
